You can't poke your eye out with a Q-tip. Well you can but it takes talent. On the other hand you can easily damage your ear drum. So by all means use Q-tips but insert them gently.
You can scratch your cornea with a Q-Tip and harm your inner ear with one, with plenty of articles available to testify to the damage that can be done. Plus, under MentalFloss, Six ways to Clean Ears without using cotton swabs.
Experiment conducted: The sound in my head that resulted from using a Q-tip in my ear was similar to sound that resulted when I shampooed my hair and scrubbed my scalp with my fingers. Neither sound seemed loud or surprising. Same with combing my hair with a plastic brush with little balls on the end. Since the illustration shows no hair on the Q-tip user's head, results may vary. Hardness of head could also be a mitigating factor.
