The problem with Gruber is that he's the only one (so far) who has been outed. His attitude of elitist, condescending sneering at the "stupidity" of the American people permeates those who authored the ACA. All of the bill's authors need to be identified and publicly questioned about why they structured the bill the way they did.
(1) I'm just going to go ahead and assume that crossing arms is just a way to bring everyone closer to one another. Same thing with shaking hands. Even more so in the age of the photo op. A theater instructor once professed that a theatrical convention is anything on the stage that consistently, hence predictably, differs from real life. That's quite a definition he had going there.
(2) I once heard it said that waving hello was a gesture that originated in proving you weren't holding a weapon. Sounds plausible enough.
(3) What is our standard? Problem is, stuff like that is easy to make up; like "fuck" is an acronym of "for using carnal knowledge." Out to dinner once, some old guy told me that "tips" stands for "to insure prompt service." He was an imbecile.
(4) That evolutionary psychology guy talked about how nearly all religions require resource-expending, submissive displays of conformity, like kneeling, or bowing, or prostrating.
He said the point isn't whether you believe in it or not. The point is that you do it in public and you can't fake it.
(5) International politicians posing for the camera might require the carving out of an exception to that general rule.
