The expression Southerners came up with, "Bless his heart" or "Bless her heart," is genius. With a seeming endearment at the end they leave no doubt the person who's been under discussion is, uh....lacking
Of course that's rarely used as a direct insult, as in "Bless your heart," But it could be.........
